Established in 1976, 5 Points Eye Associates has been serving patients with care, attention, compassion and expertise. Dr Anthony Potochick has been practicing at
5 Points Eye Associates since 2008. Dr. Potochick provides patients with a full spectrum of optometry services.
The care we give to our patients is one of a kind – and so is the 5 Points Eye Associates building. Originally constructed for the Home Telephone Company in 1914, our office at 2036 Forbes Street was designed by prominent Prairie School architect Mulford Marsh.
We can offer 1-hour service for most single vision glasses.

Our Patients Say it Best
I broke my old pair of glasses and they were able to rush me a new pair before leaving on my trip! They were all so nice and the office was clean and inviting. Will definitely be making this my new eye doctor.
Dr. Potochick gives thorough eye exams in beautiful surroundings using the latest technology. Helpful friendly staff makes the complete process a pleasure.
Had eye irritation and blurred vision. Everyone was super nice and quick. I was seen at my scheduled time and the doctor answered my questions and didn’t act rushed at all. So extremely happy! They were also cheaper and provided a lower cost script than places I went before.
Highly recommend Dr. Potochick! Competent, winsome, and thorough. His office staff is also outstanding! Joyce went the extra mile in solving an insurance issue. Wonderful patient care all the way around!
